最好的血液相容表面莫过于血管内皮细胞。有研究表明,微血管内皮细胞表面
存在着肝素样分子和其他生物活性物质,可防止局部血栓形成。如能把肝素结合在
人工材料的表面,有可能使人工材料部分模拟人体血管内皮功能,避免血栓的形成
,增加人工材料的生物相容性。1963年Gott等[1]首次采用离子结合方式将肝素
与合成材料结合,以增加人工血管的抗血栓特性。此后人们发现肝素涂层材料可有
效提高聚合材料的生物相容性,从而减轻术后全身炎症反应。
1 肝素涂层材料的原理与构造
1.1 离子结合方式 是利用肝素本身含有大量负电荷,用阳离子表面活性剂
或含有阳离子的聚合物涂抹于材料表面,通过电荷来吸附肝素分子。目前市场上的
Duraflo II表面(Baxter Healthcare Corp,Bentley Laboratories pition,
Irvine,Calif.)即是采用离子结合方式。
1.2 共价结合方式 是通过特殊共价键使肝素分子结合于材料表面。使用碳
化二亚胺把肝素连接到水解聚甲基丙烯酸甲酯上;肝素再通过戊二醛以共价键结合
在聚乙烯醇上。共价键结合需要特殊的化学反应,把肝素结合到聚合材料表面。对
CPB用品来说,每个部分材料的表面均需独立地进行肝素涂层。此种方法可使肝素
活性降低,随使用时间延长逐渐损耗,在某些情况下可能导致设备表面物理性损害
。其可能的原因是肝素作为一种粘多糖在共价键结合的过程中活性常发生改变。目
前市售Carmeda生物活性表面(Carmeda BioActive Surface, Carmeda, Stockhol
m, Sweden,CBAS)即是采用共价结合方式。
1.3 物理结合方式 是用肝素分子与聚合材料混合,在血液接触面缓慢释放
肝素分子,达到抗血栓形成的作用。
1.4 结合方式的评价 从现有的报道来看,共价键结合的材料性能可能更好
一些。研究发现,与Duraflo II相比,CBAS能显著降低常温CPB中组织纤溶酶原激
活物,补体及白细胞激活也明显减少[2-4]。Moen等[3] 研究了两种表面对血
液激活的影响。与CBAS组相比,Duraflo II组乳铁蛋白及MPO的释放明显增加;且
前者TCC形成及白细胞激活最少。关于肝素涂层管道(heparin-coated circuits,
HCC)对术后结果的影响,研究表明,Carmeda组在术后拔管时间、住院时间和节
约费用方面都显著低于Duraflo II组[5]。Ovrum等报道[6]在常规冠状动脉搭
桥术中减少全身肝素用量,使用这两种类型管道,均有良好临床效果,但两者相比
并无明显差异。
2 肝素涂层管道的临床应用
初期临床应用中仅能在膜肺上涂抹肝素。单纯应用肝素涂层膜肺(HCO)可减
轻纤维蛋白原、凝血因子XII和高分子激肽原消耗,减少血浆补体C3a和游离血红蛋
白水平,并且不影响气体交换[7]。目前已有完全肝素涂层的CPB管道、氧合器、
过滤器、插管等,使得正确评价和临床应用肝素涂层管道成为可能。
人们最初使用HCC主要是为了减少肝素用量以便于减少术后失血及需血量。研
究表明肝素本身可以引起纤溶和血小板的功能障碍[8],这些副作用是剂量依赖
性的;肝素-鱼精蛋白复合物可以通过经典途径激活补体[9]。应用肝素涂敷管道
是减少肝素用量的唯一方法,从而有效地减少肝素和鱼精蛋白带来的副作用。对于
HCC时肝素减量问题,目前尚无统一意见,然而多数作者认为使用HCC时减少全身肝
素用量是安全的。
2.1 肝素用量 关于采用小剂量全身肝素化的肝素涂层管道(LSH-HCC)时的
肝素用量及ACT时间,各家报道不一。肝素剂量在1~1.5mg/kg之间,ACT时间在150
~280s之间[10-15]。
qvrum等[12] 在CPB中所用标准为:(1)LSH-HCC:肝素:100IU/kg; ACT
>250s;(2)全剂量全身肝素化的肝素涂层管道(FSH-HCC):肝素:400IU/kg ;
ACT>480s。Seggesser等[16,17]所用的标准为:(1)FSH-HCC:体内肝素用量
为300IU/mg;预充液内为 5000IU/L;ACT>480s;(2)LSH-HCC:体内肝素用量为
100IU/mg;预充液内为1000IU/L;ACT>180s。
2.2 临床研究 研究结果表明,肝素涂层管道可减少病人术后失血和库血用
量[16,18],缩短拔管时间[30],在减少肝素用量时更明显[15-17]。应用小
剂量肝素化的HCC时,能抑制补体激活[11,13],使粒细胞激活减少[19],对纤
维蛋白原、D-二聚体、b-TG及血小板计数等方面的影响与FSH无差异[18];并能
明显减少心律失常发生率[32, 33]。病人在ICU停留时间及住院时间缩短,住院
费用减少[15]。从而抵消了使用HCC增加的费用。在儿童,HCC时病人术后中心体
温明显下降[29],与减少IL-6与循环中补体水平,降低炎症反应有关[34]。也
有报道肝素涂层管道可减轻冠脉搭桥对病人术后认知能力的损害,可能因为减少白
细胞粘附聚集于脑血管床[37]。Ranuci等[38]比较了鼓泡式氧合肺、常规膜肺
和肝素涂沫膜肺的效果。结果表明,肝素涂沫膜肺可以减轻肺的功能失常。Hamad
a等[43]报道冠状动脉搭桥手术中肝素管道与去白细胞滤器同时使用能减轻炎症
反应和肺功能的损害。Jansen[42] 研究认为,抑肽酶预充的HCC管道明显减少病
人在ICU停留时间,降低并发症的发生率,减少住院费用。
qvrum等[14]观察到LSH-HCC组的β-TG及TAT复合物在CPB结束时明显低于标
准剂量组,PF1,2、纤维蛋白原、D-二聚体及血浆纤溶酶-抗纤溶酶复合体各组无差
别;认为与FSH-HCC相比,LSH-HCC并不增加血栓形成的机会,小剂量全身肝素化的
HCC是安全的。对于凝血酶来说,目前所用小剂量肝素在允许的范围内;标准剂量
肝素对凝血系统没有任何有益的影响。
有学者对高危病人术中使用HCC进行评价[40]。并以ICU停留时间、术后住院
时间及是否出现严重并发症作为评价术后病人整体情况的指标。从全组病人或MV组
病人来看,HCC均明显改善了这些指标。McCarthy[41]探讨再次心脏手术病人使
用HCC能否改善临床结局。结果表明,使用HCC没有出现任何副作用;需要大量输血
的病人明显减少,再次换瓣病人在ICU需血量明显减少。对急、重症病人,抗凝禁
忌症的患者、不能脱机的病人需要长期机械性循环辅助者及使用ECMO者,可考虑使
用肝素涂层管道,减少甚至不使用肝素进行转流,有望获得成功[20,25]。
也有临床效果不明显的报道[14,35,36]。不同肝素涂层管道对临床影响并无
差异[6,12]。Steinberg[39]研究发现HCC对QS/Qt没有影响,可能的原因是预
充液中加了蛋白质,后者在CPB之前可以冲刷掉大部分包被的肝素。也有报道认为
再次手术HCC时并不能改善临床进程。并指出为使HCC更有利,需采取以下措施:减
少肝素用量,避免心包积血回输[28]。
一些学者批评减少肝素用量,认为这可以增加凝血酶形成和纤维蛋白微栓的危
险[26]。Cheung等[27]报道HCC时使用1mg/kg的肝素可见心内凝块形成。Mueh
rcke 等[28]在不进行全身肝素化而使用HCC行生命支持时,也发现心内普遍有血
凝块形成。很明显需要进一步研究以确定使用HCC的最安全措施。
2.3 LSH-HCC时应注意的事项 许多的研究表明,在临床上应用小剂量肝素H
CC是有效的和安全的。然而应当注意其局限性[31]。应当确认那些AT III缺乏和
血液呈高凝状态者,术中避免血液瘀滞,避免应用那些对凝血机制的平衡有影响的
药物,所有这些对避免术后出现并发症是非常重要的。
Segesser[16,17]等指出,小剂量肝素应用指征为病人在标准肝素化有危险
时,此时必须有合适的外科技术和严格的灌注条件方能保证手术的安全。小剂量肝
素化的HCC时,要注意以下三种情况:⑴ 全流量:> 2L/min,连续数h仍安全;⑵
低流量:1 ~ 2L/min,短时间(数分钟)内是安全的,如果需要延长低流量时间
,应同时延长ACT时间;⑶ 微流量:< 1L/min, 能维持数秒钟,尽量避免出现这种
情况;停循环其间应全量肝素化。
小剂量肝素化HCC时有可能出现的问题及解决办法见下表。
Dowling等[20] 认为,在无全身抗凝的情况下,使用HCC应注意以下几个方
面:⑴ 术前AT III需正常;⑵ 插管后即转流;⑶ 保持CPB高流量:2.5L/min;⑷
注意出血及引流血的抗凝回收;⑸ 不用滚压泵吸引器。

3 结论
肝素涂层管道是提高生物相容性的有效方法之一,可减轻由于血液与异物接
触引起的全身炎症反应,提高转流过程的安全性,其临床意义正在得到越来越多学
者的认可。尤其作为减少甚至不使用肝素的唯一方法,可减少应用大剂量抗凝剂带
来的并发症,在某些特殊情况下,发挥了令人满意的作用。但肝素涂管道并不能达
到理想的生物相容性,仍具有一定的局限性。随着对肝素涂层管道研究的不断深入
,产品设计和制造工艺的不断提高,最终肝素涂层管道会在临床上得到广泛的应用
。
