简介:AIM:Todescribetheresultsof5consecutivecasesofclearlensextractioninangleclosurepatientsforthetreatmentofelevatedintraocularpressure(IOP).·METHODS:Retrospectiveobservationalcaseseries.Allangleclosurepatientswereontwoormoretopicalglaucomamedicationsandhadpriorlaseriridotomy.Eyesunderwentclearlensextractionbyphacoemu-lsificationwithintraocularlensimplantation.·RESULTS:Allfivepatientsinthiscaseseriescarriedthediagnosisofangle-closureglaucomaandhaduncontrolledIOPpriortosurgerydespitetopicalmedications.AfterclearlensextractionthreeofthecaseshadgoodIOPcontrol(IOP<22mmHg)withouttheneedfortopicalmedications.InonecasetheIOPwasbettercontrolledaftersurgery,however,topicalmedicationswererequired.ThedesiredIOPwasnotmetinonecasedespiterestartingmaximumtopicaltherapy.·CONCLUSION:Thiscaseseriessuggeststhattheremaybearolefortherapeuticclearlensextractioninselectcasesofangle-closureglaucoma.

简介:ObjectiveToreportexperienceswithuseofotoendoscopyincerebellopontineangle(CPA)surgeries.MethodsTwentyfivecasesofCPAsurgeriesperformedbetweenNovember2002andDecember2008inwhichmicroscopeenabledotoendoscopywasusedwerereviewed.The25casesincluded19casesofacousticneuroma,3casesofCPAfacialnervetumors,1caseoftrigeminalneurinoma,acaseofglossopharyngealneuralgiaand1caseofhemifacialspasm.Endoscopywasusedinallcasestogetherwithmonitoringofbrainstemauditoryresponsesandfacialelectromyography.Postoperativehearingandfacialnervefunctionwereevaluatedandcomparedtopre-operativelevels.ResultsEndoscopyprovidedimprovedvisualizationoflocalanatomy,revealedhiddenlesionsandreducedunnecessaryanatomicaldistortions.Totalresectionwasachievedin18ofthe19acousticneuromacases,Facialnerveanatomicalintegritywaspreservedinall19cases.OneweekpostoperativeHouse-BrackmanngradingwasIin3cases,Ⅱin10casesandⅢin6cases.Facialnervefunctioncontinuedtoimproveinsomecasesat3months.Totaltumorresectionwasachievedinall3patientswithfacialneurinoma.Thefacialnervewassacrificedin2ofthe3caseswithprimaryfaciohypoglossalnerveanastomosis.FacialnervefunctionwasGradeⅡandGradeIIIoneyearaftersurgery,respectively.Inthecasewithanatomicallypreservedfacialnerve,postoperativefacialnervefunctionwasinitiallyGradeⅢandimprovedtoⅡat3months.ThetumorwascompletelyresectedinthetrigeminalneurinomapatientwithaGradeⅢpostoperativefacialnervefunctionwhichimprovedGradeIIthreemonthslater.Seventeenofthe19patientswithacousticneuromaretainedhearingpostoperatively,ofthese12maintainedpreoperativelevelsofhearing.Preoperativehearingcapacitywaspreservedin2ofthe3patientswithfacialnervetumors,butlostinpatientswithothertumortypes.Glossopharyngealneurotomy(n=1)andmicrovasculardecompression(n=1)resultedinsatisfactorysymptomrelie
简介:ObjectivesToexplorethefeasibility,necessity,andsecurityoftranscatheterclosureofpatentductusarteriosus(PDA)ininfants.MethodsTherewere230infantswithPDA.Theageswere(7.3±3.2)monthsandtheweight(6.6±2.8)Kginaverage.Theywereseparatedintotwogroups.GroupAwasformedbytheinfantsweighinglessthan6Kg,GroupBover6Kg.RightheartcatheterizationwasperformedfirsttocalculatetheratioofQp/Qs.ThendescendingaortographydemonstratedthediameterandshapeofPDA.Properoccluderwasselectedtofinishtheintervention.Echocardiographywasperformedafterintervention24hoursand1,3,6,12and24months.ResultsInGroupAthetechnicalachievementratiowas94.6%withtheaveragediameterofPDA(6.2±3.2)mm.InGroupBthetechnicalachievementratiowas100%withtheaveragediameterofPDA(4.8±2.5)mm.WeusedtheAmplatzerDuctOccluderwiththetypefrom6-8mmto12-14mm,thedeliverysheathfrom6Frenchto8French.24hoursafterintervention,echocardiographydemonstratedthattherewere6residualshuntsinGroupAwhile22inGroupB.After1year,residualshuntexistedinneithergroup.Therewere4patientswhosefemoralarteriespulsedweaklyafterinterventioninGroupA,whileinGroupBtherewere3.Theyallrecovered24hoursaftertheapplicationofurokinase.InGroupAbloodflowvelocityindescendingaortaincreasedin5infants,whileinGroupBtherewere3.Theyallresumedin6-12months.ConclusionsTranscatheterclosureofPDAininfantsissafeandtechnicallyfeasible.However,indicationshouldbestrictlyselectedandtheinterventionshouldbeperformedbyexperiencedphysician.
简介:Inthisstudy,thesampledatawasbasedon2790samplesof9treesfromdahurianlarch(Larixgmelinii)plantationslocatedinQitaiheForestBureauinHeilongjiangProvince.ThemodifiedLogisticmodely=b1/[1+exp(b2x)]+b3wasselectedtomodelingmicrofibrilanglefromsixmodelsbasedonnonlinearregression.Then,thelogisticmodelwasfittedusingnonlinearmixed-effectmodelingapproachbasedonNLMEofS-PLUSsoftware.Evaluationstatistics,suchasAIC,BIC,LogLikelihoodandLikelihoodratiotestwereusedformodelcomparisons.TheresultsshowedthattheLogisticmodelwithparametersb1,b2,b3asmixedeffectsshowedthebestperformance.Correlationstructuresincludedcompound-symmetrystructure(CS),first-orderautoregressivecorrelationstructureAR(1),movingaveragecorrelationstructureMA(1)andautoregressive-movingaveragecorrelationstructure[ARMA(1,1)]wereincorpratedintothebestmicrofibrilanglemixedmodel.[ARMA(1,1)]significantlyimprovedtheprecisionofmixedmodel.Validationconfirmedthatthemixedmodelwithcalibrationofrandomparameterscouldprovidemoreaccurateandpreciseprediction.Therefore,theapplicationofmixedmodelnotonlyshowedthemeantrendsofmicrofibrilangle,butalsoshowedtheindividualdifferencebasedonvariance-covariancestructureandcorrelationstructure.
