简介:Thispaperreviewscurrentrecommendationsontheappropriateevaluationandmanagementofcardiacarrhythmiasinthepregnantpatient.Mostarrhythmiasduringpregnancyarebenignandrequirenointervention.Whenrequired,thedecisiontotreatshouldbebasedonsymptomseverityandtheassociatedrisktomotherandfetusposedbypotentiallyrecurringarrhythmiaepisodesthroughoutthepregnancy.Anytreatmentstrategyinthispatientpopulationhasinherentrisktobothmotherandunbornchild.Beforetheinitiationofanyintervention,documentationofaclinicalarrhythmiaandcorrelationwithclinicalsymptomsshouldbeobtained.Thereisnoroleforempirictherapy.

简介:Tocontrolmovement,thebrainhastointegrateproprioceptiveinformationfromavarietyofmechanoreceptors.Theroleofproprioceptionindailyactivities,exercise,andsportshasbeenextensivelyinvestigated,usingdifferenttechniques,yettheproprioceptivemechanismsunderlyinghumanmovementcontrolarestillunclear.Inthecurrentworkwehavereviewedunderstandingofproprioceptionandthethreetestingmethods:thresholdtodetectionofpassivemotion,jointpositionreproduction,andactivemovementextentdiscrimination,allofwhichhavebeenusedforassessingproprioception.Theoriginofthemethods,thedifferenttestingapparatus,andtheproceduresandprotocolsusedineachapproacharecomparedanddiscussed.Recommendationsaremadeforchoosinganappropriatetechniquewhenassessingproprioceptivemechanismsindifferentcontexts.
简介:Lymphomararelyoccursduringpregnancy,makingthisconditiondifficulttodefine.Lymphomasthatoccurinreproductiveorgansduringpregnancyexhibituniqueclinicalcharacteristics.Amongthelimitedcases,non-Hodgkin'slymphoma(NHL)showsaconsiderablyhigherincidenceratethanHodgkin'slymphoma(HL);NHLalsodisplaysclinicalcharacteristics,suchashighaggressiveness,advancedstage,andpooroutcome.Thisstudyreportsonfourcasesoflymphomasinnon-gonadalorgans(HL,n=2;NHL,n=2)duringpregnancy.Thetumorsrapidlyprogressedinallpatientsduringpregnancybutremittedattheendofpregnancyand/ortherapy.ThetwoHLcaseswerenodularsclerosisclassicalHLandtreatedwithchemotherapyafterterminatingthepregnancy.OneoftheNHLcaseswasprimarycutaneousfollicularcenterlymphoma,aBcell-derivedindolentlymphoma.Thepatientwasfollowedupwithoutanytherapyafterterminatingherpregnancy.Theothercasewasafollicularlymphomagrade3B,whichwastreatedwithchemotherapyafterdelivery.Wealsoconductedaliteraturereviewof165lymphomacasesoccurringduringpregnancyreportedfrom1976to2013torevealthecorrelationbetweenpregnancyandlymphomaprogression.Immunohistochemistrystudieswereperformedtodeterminetheexpressionofestrogen/progesteronereceptors(ER/PR),andERwasweaklypositiveandsporadic.Weconcludedthatlymphomasoccurringduringpregnancyshouldbemanagedwithapromptandreasonabletreatment.Highestrogenlevelinmaternalbodymayaffectlymphomaprogression.
简介:Hearinglossisaconditionaffectingmillionsofpeopleworldwide.Conductivehearingloss(CHL)ismainlycausedbymiddleeardiseases.ThelowfrequencyareaisthepivotalpartofspeechfrequenciesandmostfrequentlyimpairedinpatientswithCHL.AmongvarioustreatmentsofCHL,middleearsurgeryisefficienttoimprovehearing.However,variablesuccessratesandpossibleneedsforprolongedrevisionsurgerystillfrustratebothsurgeonsandpatients.Nowadays,increasingnumbersofresearchersexplorevariousmethodstomonitortheefficacyofossicularreconstructionintraoperatively,includingelectrocochleography(ECochG),auditorybrainstemresponse(ABR),auditorysteadystateresponse(ASSR),distortionproductotoacousticemissions(DPOAE),subjectivewhispertest,andopticalcoherencetomography(OCT).Here,weillustrateseveralmethodsusedclinicallybyreviewingtheliterature.

简介:PolysaccharidesfromnumeroustraditionalChinesemedicineshavebeenprovenasthebioactiveingredientsandarehenceusedasthequalitycontrolmarkers.However,theassessmentcriteriaalwaysshowapoorspecificity,duetothelackofmaticcomparisonamongtheanalogousherbs.Inthepresentstudy,twosimilarmaterials,namelysea-tangleandsargassum,wereselectedasthemodelherbstodevelopmorespecificmethodsforqualitycontrol.Twowell-establishedmethods,determinationofthetotalpolysaccharidescontentandmonosaccharidescompositionanalysis,werebothemployed.Baseduponthequantitativeresults,theevaluationcriteriaofthepolysaccharidescontentsofnotlessthan2.0%and1.7%wereproposedforsea-tangleandsargassum,tively.NineidenticalmonosaccharidederivativesappearedontheHPLCchromatogramsofthehydrolysisandderivatizedsolutionsofthetwodrugs.Principalcomponentanalysisandorthogonalpartialleastsquaresdiscriminantanalysisusingthepeakareasofmonosaccharidesderivativesasthevariableswereperformed,andtheresultsindicatedthatmannuronicacidandxylosewiththeoppositeconcentrationsinthetwodrugswerethedifferentialcomponents.Adiscriminativecriterionusingthepeakarearatioofthesetwomonosaccharidesderivativeswasproposedforthequalitativeidentification.Inconclusion,amorespecificandquantitativequalitycontrolmethodwasdevelopedforsea-tangleandsargassum.
