简介：摘要目的探讨内镜下异物取出术中应用软性外套管的优势及其护理配合.方法回顾性分析在２００５年１月至２０１３年１２月期间因怀疑有上消化道异物而在我院就诊的１２６例患者,比较应用不同内镜检查治疗方法后的并发症率、成功率和术中护理配合的方法.结果对应用软性外套管治疗的３９例患者中,通过术中与医生密切配合,异物成功取出率为１００％,取出过程中出现食管轻度粘膜损伤的患者１例,为２．５６％.８７例患者接受传统内镜检查,治疗和护理方法,成功取出异物的患者６７例,成功率为７７．０１％,未能取出异物的患者２０例,为２２．９９％;将食管内异物推送入胃腔中为４例,发生嵌顿的有１３例,予外科手术的３例;传统内镜检查和治疗方法中食道上段粘膜发生出血及损伤的患者２０例,为２２．９９％.异物取出术使用内镜软性外套管成功率为１００％,并发症发生率为２．５６％;异物取出术未使用内镜软性外套管辅助的成功率为７７．０１％,并发症发生率为２２．９９％.结论在应用软性外套管辅助治疗的异物取出术中,通过与医生的密切配合和精心细致的护理,能有效提高异物取出的成功率,是降低食管上段粘膜损伤,减少并发症产生的关键.该方法的使用能有效的减轻病人痛苦,简单,易行.关键词软性外套管;消化道异物;内镜;护理EndoscopicforeignbodiesinapplicationtheadvantageofsoftoutercasingandnursingcooperationYanCaoHaiBoLangjiezhang?(Beijinganzhenhospitalaffiliatedtothecapitaluniversityofmedicalsciences,digestiveinternalmedicine１０００２９)AbstractObjectivetostudytheendoscopicforeignbodiesinapplicationtheadvantageofthesoftoutercasingandnursingcooperation．MethodsaretroＧspectiveanalysisinJanuary２００５toDecember２０１３inourhospitalbecauseofsuspectedofuppergastrointestinaltractforeignbodiesofthe１２６patints,comparetheapplicationofdifferentendoscopicexamination,successrateandcomplicationrateafterthetreatmentmethodsofintraoperativenursingcooperation．Resultsthesoftoutercasingtreatmentof３９cases,throughintraoperativeinclosecoordinationwiththedoctor,foreignbodyremovedsuccessfullyatarateof１００％,apＧpearedintheprocessoftakeoutpatientswithesophagealmildmucosalinjuryin１case,２．５６％．８７patientsreceivingtraditionalendoscopicexamination,treatＧmentandnursingmethod,successfullyremoveforeignbodiesofthe６７casesofpatients,thesuccessratewas７７．０１％,２０patientswithfailedtoremoveforeignbody,２２．９９％;Esophagealforeignbodypushintothestomachcavityfor４cases,thereare１３casesofincarceratedin３casesofsurgery;Traditionalendoscopicexaminationandtreatmentofesophagealmucosahemorrhageoccursintheupper２０casesofpatients,anddamageof２２．９９％．Foreignbodiesusingendoscopicsoftoutsidecasingsuccessratewas１００％,theincidenceofcomplicationswas２．５６％．Foreignbodiesnotusingendoscopicsoftoutsidecasingauxiliarysuccessratewas７７．０１％,theincidenceofcomplicationswas２２．９９％．Conclusiontheapplicationofsoftauxiliarytreatmentofforeignbodiesinoutercasing,closelycooperatewiththedoctorandcarefulandmeticulouscare,caneffectivelyimprovethesuccessrateoftheforeignbodiesistoreduceesophagealmucosallesionsintheupper,thekeKyetyowreodrduscingcomplications．Theuseofthismethodcaneffectivelyreducethepatientpain,simple,andeasy．Softoutercasing;Thedigestivetractforeignbodies;Endoscopy;Nursing中图分类号R６５６．６文献标识码B文章编号１００８－６３１５(２０１５)１０－０３３６－０２
