摘要
摘要目的探究在无痰、痰菌阴性不典型肺结核诊断中应用纤维支气管镜的临床效果.方法对２０１４年１０月至２０１５年１１月在我院检查并确诊为无痰、痰菌阴性不典型肺结核５０例患者的临床资料进行回顾和分析,为方便研究,将此５０例患者作为观察组,选择同期到我院治疗的非结核肺部疾病患者５０例,将其作为对照组,用纤维支气管镜对两组患者进行检查,观察其镜下表现及各项检测指标.结果观察组纤维支气管镜刷检阳性率为４２％,灌洗液快速培养阳性率为６２％,１８例患者需要活检,阳性率为４４％,对患者纤维支气管检查后的痰涂片进行检查,阳性率为３０％,对患者的镜后痰液进行快速培养,阳性率为５２％.对照组患者均未发现结核证据.结论在无痰、痰菌阴性不典型肺结核患者的诊断中,应用纤维支气管镜能够实现良好的临床效果,提高确诊率,具有很大的推广使用价值.关键词纤维支气管镜;不典型肺结核;诊断;临床效果abstractobjectivetoexploreintheabsenceofphlegm,phlegmgermfeminineatypicaltuberculosisdiagnosisinapplicationoffibergaswithoutphlegm,theclinicaleffectofthemirror．MethodsinOctober２０１４toNovember２０１５tothehospitalcheckanddiagnosisofnophlegm,sputumbacteriaof５０casesofatypicalpulmonarytuberculosispatientswithclinicaldatareviewandanalysis,fortheconvenienceofstudy,the５０patientsasobservationgroup,andthenselectatthesametimetothehospitaltreatmentof５０casesofpatientswithlungdisease,tuberculosisputitasacontrolgroup,withfiberbronchoscopeinspectionontwogroupsofpatients,observethemicroscopicperformanceandindicators．Resultsobservationgroupoffiberbronchoscopebrushtestpositiveratewas４２％,lavagerapidculturepositiveratewas６２％,１８patientsneedabiopsy,positiveratewas４４％,ofthefiberopticbronchoscopeexaminationinpatientswithsputumsmearcheck,positiveratewas３０％,torapidlydevelopinpatientswithsputum,thepositiverateis５２％．HasnotbeencontrolgrouppatientswithsymptomsofTB．Conclusionintheabsenceofphlegm,sputumbacterianegativediagnosisofatypicalpulmonarytuberculosispatients,applicationoffiberbronchoscopecanaＧchievekgeyoowdocrldisnicaleffect,increasetherateofdiagnosis,letpatientsreceivingtreatmentatanearlydate,greatlypromotetheusevalue．fiberopticbronchoscopy．Atypicaltuberculosis;Diagnosis;Clinicaleffect中图分类号R５２１文献标识码B文章编号１００８－６３１５(２０１５)１２－１３１８－０２
